North Carolina's DHSR lists 78 licensed adult care homes — the state's assisted living license — across Mecklenburg, Cabarrus, Gaston, Union and Iredell counties, with 5,807 licensed beds in total (Mecklenburg 39, Cabarrus 9, Gaston 11, Union 7, Iredell 12). Every one is shown below with its license number, bed count, address, phone and current DHSR star rating.
47 of them also hold Special Care Unit (SCU) beds for Alzheimer's and dementia care, and 37 of the largest have a full profile page with their inspection and penalty history. Smaller 2-6 bed homes are licensed separately as family care homes.

DHSR issues every adult care home a 0-4 star rating from its inspection scores: four stars requires 100+ points on two consecutive annual inspections, three stars is 90-99.9 points (or one 100+ survey), two stars 80-89.9, one star 70-79.9 and zero stars 69.9 or lower. In this metro, 26 of 78 rated adult care homes currently hold four stars.
Ratings come from DHSR and county DSS inspections, not from us — see the DHSR star rating scale. Always read the numeric score and the date: a rating issued after a follow-up or monitoring visit can move up or down between annual surveys.
21 of the 78 metro adult care homes have at least one DHSR administrative penalty imposed in the last 36 months under G.S. 131D-34; each featured profile lists the rule cited, violation level, amount and status, and links to the state inspection report.
A penalty is a serious-violation finding (level A1/A2 or an unabated violation), so it is worth asking the administrator what changed. The full statewide list is on DHSR's penalty page.
In North Carolina both are delivered under the same DHSR adult care home license; memory care is a Special Care Unit (SCU) inside a licensed adult care home with extra staffing, dementia training and disclosure rules, not a separate license.

Adult care home assisted living in Greater Charlotte typically runs $4,200-$5,800/mo in 2026; Ballantyne/South Charlotte, the Lake Norman towns and Waxhaw run higher, while Gastonia, Statesville and parts of east and west Charlotte run lower.
